Obituary For Jack Chaplin Densmore

November 3rd, 2003 – September 12th, 2021

It is with great sadness that the family announce the passing of Jack Chaplin Densmore. After a long battle with mental health issues and despite efforts to ease his suffering by family, friends, and healthcare professionals, Jack succumbed to his illness at the age of 17.

He is survived by his parents Sarah and Matt, sisters Emme and Zoe, and grandparents Jan and Wayne Densmore, Kim MacKenzie and Rob Nash, Barry and Judy MacKenzie, and a large extended family that includes many aunts, uncles, and cousins.

A memorial for Jack will be held on Friday, September 24th at 11am at the Woodlawn Library in Dartmouth followed by a small reception and interment at Dartmouth Memorial Gardens from 1pm-5pm. Due to COVID-19 restrictions, seating is limited at the library and attendees will be notified prior to the event. For those unable to attend the memorial, the interment is open to the general public and starts at 1pm.
